The app downloads every form the account can see on this server. For KoboToolbox use the KoboCAT host (kc.…), never the kf./eu. web interface.
Your own KoboToolbox (kc. host) or any OpenRosa-compatible server. For KoboToolbox forms that accept anonymous submissions, use https://kc.kobotoolbox.org/<owner username> and leave the account empty.
The account travels inside the QR code: whoever scans it is logged in without typing anything. Use an account that only has "add submissions" on the forms of this team.

Name, letter and colour shown at the top of the app, so a supervisor can tell from afar that the device is on the right project.
Protects the app's settings menu. Without it anyone can undo the locks of step 5. Keep it with the coordination team; it also travels inside the QR code.
The default is the field device: the interviewer opens the app, picks the form and fills it in; sending and form updates happen on their own and the settings stay locked.
On = the interviewer may use it. Off = hidden behind the admin password.
